Use original printable tools to track youth-service documents, preserve credential claims and map provider communication procedures.

These original worksheets help a family read one packet carefully. They are designed for private use with the documents already supplied by the provider. They do not create a provider certification or determine a youth's clinical needs.

Tool 1: the document index

Create one entry for each file, webpage or email. Preserve its original title and wording so a later response can be connected to the correct item.

Tool 2: the scope and credential card

Use a separate card for each role or qualification you need to understand. Ask the issuer for an explanation when a description does not identify who performs the service.

Tool 3: the communication map

Record the procedure the organization actually supplies. Separate routine questions from records questions and urgent concerns; do not assume one channel answers every type of request.

HHS's model-notice page was reviewed February 20, 2026 and links the revised notice models. Read it as a primary reference, then ask the organization which current notice applies to its service. The worksheet organizes that question; it does not decide the applicable privacy rules.
